Our unique program offers individualized services to each Participant in our program
The Connections Adult Day Services Program is an inclusive activity based program designed to develop and enhance skills and new learning opportunities in social, emotional and person development and community integration. The Connections Adult Day Services Program offers many programs including but not limited to, educational classes, clubs, science, technology, cooking, crafting, gardening, physical activity and sensory stimulation. Individualized outcomes are created for each participant through person centered planning strategies, which are reassessed on an ongoing basis. Person Centered Plans address how a person’s individualized goals can be met through involvement in activities both inside and outside of the setting. The Connections Adult Day Services Program provides opportunities for regular meaningful non work activities in integrated community settings for the amount of time desired by the participant. The setting includes non-disability specific locations in providing services such as volunteer opportunities in the community or engaging in integrated community activities, such as health clubs, senior centers, interest group activities and/or faith based activities.
The Connections Adult Day Services Program provides an integrated physical environment conducive to meeting individual outcomes. There is an indoor gathering space, outdoor gathering space, large group activities area, small group activities area, private space, area for calming activities and area for stimulating activities. The setting is able to adapt activities and schedules to the needs and preferences of people who receive support and provides a variety of activities throughout during the day. The Connections Adult Day Services Program uses adaptive aids and technology to assist people to participate in activities as well as offers an accessible environment to promote maximum participation among people. Social interaction is encouraged for everyone attending the program. Staff support interaction among participants by facilitating conversations and providing space for uninterrupted interactions. When planning events and activities, The Connections Adult Day Services Program makes an effort to match people with like interests and considers preference in who people want to spend their time with. The Connections Adult Day Services Program seeks out opportunities to match tasks and activities to people’s age, skills, abilities or desires/goals for people receiving services.
Community engagement and interaction is a priority for The Connections Adult Day Services Program. People from the community have daily interaction with the people participating in the Connections program. These people include people without disabilities doing similar things, volunteers, family and friends who are not paid to provide support. In location, the Connections Adult Day Services Program is housed within the Connections Community Center and offers easy and convenient access to the broader community. The Connections Adult Day Services Program often supplements, however is separate from medical, behavioral or therapeutic services.
Person Centered Planning means supports and services begin with dreams, goals, strengths and abilities. Based in the belief that everyone has the capacity to contribute, the person centered planning at the Connections Adult Day Services Program, builds and sustains relationships while providing opportunities for new learning through meaningful choices and self-direction. The Person Centered Planning approach utilized by the Connections Adult Day Services Program puts people first and values all people.Â
