Individualized Support for “D”

Hello everyone,

I completed my yearly casemanagement assessment with D on the phone yesterday, in preparation for his doctor appt tomorrow. I want to commend everyone for the high level of face-to-face support that Opp Inc is giving D. He explained that he is given FOUR face to face staff contacts a day, at each of his med times-THIS is what is making all the difference in his life, particularly ensuring that he is taking all of his medications, not missing doses, and correct use of his insulin, testing his BS’s, etc.

The medication oversight, in addition staff face to face involvement of his ADL’s, taking/sitting in on his multiple medical appointments with staff making sure that he follows the recommendations, and last but not least, the great socialization activities that he and his OPP Inc. staff engage in. WOW!  I do not think I have seen him this spontaneous, hopeful, clear minded in at least 10 years! One would never know he is carrying that vascular dementia diagnosis, along with his blood sugar issues…(insulin dependent), etc. Activities that he excitedly shared included frequent fishing, walking, going to the Farmers Market, etc. etc. etc. Stated he even received staff assistance in going to the area Cemetery around Memorial Day. AND THEN THERE IS HIS BIKE! He explained in detail that he has every bell and whistle on it one can imagine, and he is SOO excited about it!
