Person Centered Planning and Informed Choice is our Priority

Person Centered Planning means supports and services begin with dreams, goals, strengths and abilities.  Based in the belief that everyone has the capacity to contribute, the person centered planning at the Connections Adult Day Services Program, builds and sustains relationships while providing opportunities for new learning through meaningful choices and self-direction.  The Person Centered Planning approach utilized by the Connections Adult Day Services Program puts people first and values all people. 

The Connections Adult Day Services Program is committed to listening to the individual to identify choices and preferences they have for their lives. Connections staff are trained in the process of listening and learning to understand a person capacities and choices.  The Connections Adult Day Services Program provides opportunities for people to practice informed choice.  Choices are offered in the areas including but not limited to:

  • Activities, classes, clubs and community engagement
  • How to spend the day
  • Outcome development
  • Friends and support systems
  • Staffing preferences are taken into consideration, but not guaranteed
